Community Service Log

Park High requires students to complete 40 hours of community service for graduation.  The Link below will take users to a forced copy of the Community Service Hours Tracking Form.  Students need to make a copy of the form and fill it out as they complete the service hours. 

 

Students should retain this file for the four years of high school to turn in when complete.  This tracking form is also very useful for scholarship applications.
