Montana requirements for immunizations in different grades.
A Medical Exemption form must be filled out and signed by a medical doctor if there is a medical reason a student cannot be immunized. This form must be submitted in order for a non-immunized student to be enrolled in school.
A Religious Exemption form must be signed and notarized yearly by the parents, for those students who have a religious reason that they cannot be immunized. This form must be submitted in order for a non-immunized student to be enrolled in school.

I graduated from Middlebury College in Middlebury, VT with a B.A. in Biology/Psychology. After teaching high school biology for two years, I returned to school to get my RN/MSN degree as a Family Nurse Practitioner from MGH Institute of Health Professions in Boston, MA. I worked as a Nurse Practitioner for 10 years before taking some time off to raise our three children. I am thrilled to be back working in healthcare as a school nurse. My family and I enjoy the great community of Livingston and exploring the wild lands which surround us.

I attended Montana State University and graduated with a bachelor of science degree in nursing in 1986. I worked in various fields of nursing until I started working as a school nurse in Livingston Montana in 2001. We love Livingston, and loved raising our three children here.
